We are looking for a dedicated and compassionate Home Health LPN to join our team. This individual will provide highly specialized skilled nursing services in patient homes, under the direction of the plan of care.

How You Will Make An Impact

Provide ongoing activities pertaining to client needs and appropriate services
Evaluate effectiveness of care plan and treatments, reporting findings to physician and agency Director of Nursing
Receive and transcribe physician's orders
Provide for the physical and emotional comfort and safety of each client
Notify physician and agency Director of Nursing of unusual reactions and/or changes in a client's condition or living situation
Document all appropriate observations and treatments in keeping with agency policies and procedures
Educate client and/or family in disease process, prescribed treatments, side effects of medications, treatments and any complications requiring medical intervention
Provide monthly summary of services and client outcomes to physician and agency supervisor
Work as an integral part of the client's care team to ensure safe, effective delivery of high-quality services
Attend team meetings and case conferences as assigned
Exude professionalism in all situations

What We Are Looking For

Current, unencumbered Florida LPN license
1 year of current experience
CPR certification
IV certification
