At time of your appointment, show provider your Nomi Health ID.
Enrolled in a PPO, EPO, or HMO Plan? You will have no out-of-pocket costs for your bundled procedure.
Enrolled in a HDHP w/ an HSA? You will pay 100% of the pre-negotiated, transparent rate for services through our program until you have satisfied the yearly IRS minimum. Once your yearly minimum has been satisfied, you will have no out-of-pocket costs for services.
*Nomi Health is a benefit designed to be a no-cost offering to members. The IRS requires a federal minimum deductible for designated HSA qualified HDHP health plans. With these plan types, Employers cannot provide first-dollar coverage prior to the member meeting their federal minimum deductible ($1,500 individual plans/$3,000 family plans for 2023) - Not sure what plan type you have? For any specialty service such as surgeries, x-rays, advanced imaging (MRI, CT), physical therapy, labs and hundreds more. Use your Nomi Health digital ID card when using a participating facility.
